What are the buttons?
A radio button  means only one answer maybe selected

A check box  means more than one answer is allowed.

Click on the Next Button   to go to the next question.

Click on the Back Button  to go to the previous quesiton.

Q: Why do I get the message "must have a response" when I have typed in an answer in the text box?

A: You must select the "other (specify)" option in the list of answers
